what is a Clinomics ... watch clinic?
watch combines the advanced work-flows of the ACT clinic with the patented functionality of the category based communication structure of Clinomics ... . It allows the healthcare team to monitor specific health parameters while patients are at home. These may be simple clinical parameters such as blood pressure or HbA1C or more complex clinical information such as reporting symptoms after being discharged following abdominal surgery. It is especially helpful for conditions which require serial observations such as a cellulitis, a healing fracture, or starting a new medication. It allows for efficient, cost effective care that improves quality, while respecting the time of our patients.

Rural and community based physicians can access Clinomics... specialists simply by messaging or faxing a consult to the Acute Care Transition Clinic (ACT). This can be simply for advice on a patient that they are managing in the community or to add a patient to the specialist's virtual clinic where they can actively participate in the management of that patient's condition. For example, surgeons can assess the need for non-emergent surgical conditions and discuss the pros and cons of surgical intervention. This may be particularly helpful for patients experiencing geographic or mobility challenges. Similarly, Internists may choose to manage a patients chronic disease virtually so as to keep a closer eye on them via a "watch" clinic.
